Sign in with your Ä¢¹½ÊÓƵ Portal credentials.

Click the menu bar (grey bar with three blue lines).

Click on “Forms”.

Scroll down to “Ä¢¹½ÊÓƵ Patient Forms” and complete the following forms: Ä¢¹½ÊÓƵ Medical History, Ä¢¹½ÊÓƵ Privacy Policy Acknowledgement, Ä¢¹½ÊÓƵ Release Authorization, and all other forms related to your visit (Ä¢¹½ÊÓƵ Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)
